This project is a collaborative effort between, LFCS, County Public Health, El Concilio, CUFF and Su Salud. Health Access is a health referral project. Outreach workers work closely with the community and refer them to health care providers. The project also offers free transportation for anyone needing transportation to their appointed meetings with their health care providers. Health Access provides a mobile health vehicle to the community. This mobile health vehicle will have staff of nurses and a physician.
