CalWORKs Case Management Services is a collaborative effort between LFCS, Inc., Lao Khmu Association, VIVO, Cambodian Community, and Indo-Chinese Association. School Readiness Initiative

The School Readiness Program serves the Westwood, Clairmont, Madison and Cleveland School . This program focuses on getting children 0-5 years of age ready for school by reaching and bonding with children during home visits. Services Includes: Early Care and Education, Parenting and Family Support, Health and Social Services, School Capacity/Referrals.

Mental Health Services

Providing outreach and educational services to members of the Hmong community including Children and Youth, Adults, and Older Adults on mental health and educating the availability of services.   Establishing a network of contacts, and identifying underserved person with serious mental illness.   Conducting workshops and education sessions, community outreach to clan leaders and elders, shaman and others in the Hmong community, and prepare and distribute broad-based outreach materials to Hmong language media, places of worship and businesses, etc. Contact: Khoua Yang, MSW, Case Manager

 

Health Access

This project is a collaborative effort between LFCS, Inc., Public Health Services, El Concilio, CUFF and St. George.  Health Access is a health referral project.  The project also offers free transportation for any one needing transportations to their appointments with their health care providers.  Also, Health Access has a mobile health vehicle to provide checkups for the community.  This mobile health vehicle has a staff of nurses and a physician Contact:  Pa M. Specials Youth Project (TCWF)

To support development and implementation of school-based sexuality education program target Southeast Asian youth in Stockton . Phase II of the Hmong Resettlement Health Project

Our portion of the Hmong Resettlement Health Project focuses on achieving system change by engaging policymakers with the Hmong population, and teaching and helping all the other project collaborative partners do the same. In addition, we are also proposing capacity-building activities in order to ensure the long-term success of Lao Family Community of Stockton so that we can remain the leading provider of services and advocacy for Hmong people in San Joaquin County . The bulk of our project is designed to increase the language access and cultural competence of health systems by helping Hmong community organizations throughout California engage their local policymakers – and state and federal policymakers – on health and health-care issues. We will do this by providing training to the collaborative partners on topics including health advocacy, community organizing and civic engagement. Contact: Dr. Terry Yang, Community Organizer
